Our nursing and family support prevention and early intervention services help families become as strong and healthy as possible through research- and relationship-based care.
Our goal is to engage with families as early as possible, preferably during pregnancy, and support the whole-family care and connection that forms the building blocks of a child’s healthy development. We offer skilled nursing services for pregnant and postpartum people.
Our Wellness Program provides services for pregnant/postpartum individuals and families with children up to six years of age to support healthy relationship development and address stressors like physical and mental health issues, intimate partner violence, housing instability and food insecurity.
We serve a high proportion of refugees and immigrants and are engaged with community organizations dedicated to identifying health inequities and improving access to needed care and services.
Our pediatric skilled nursing services include assessment and care for acutely and chronically ill children from birth to age 18. These services include intravenous (IV) therapy, wound care, management of chronic diseases, oncology and other medical services.
Our pediatric rehabilitation team provides physical, occupational, and speech-language therapies for children in the comfort of their home so they, and their families, can thrive.
Services include assessment and interventions for children with developmental delays, as well as acute and chronic illnesses.
Our Pediatric Rehabilitation therapists work closely with primary care providers and Vermont’s Early Intervention program to support children and their families as early as possible for the best health outcomes.
Like our Nursing and Family Support services, our relationship-based therapeutic care offers children and their families an opportunity to receive the care they need with caregivers they trust, without even leaving home.
